Step 2: Select Your Service and Appointment Date
Next, choose the type of service you need. Whether it's a routine check-up, a medication refill, or a consultation, we've got you covered. After selecting your service, pick a date that works best for you from the available options.
Step 3: Continue as a Guest and Complete the Form
You don't need an account to book an appointment. Simply continue as a guest and fill out the necessary information in the form provided. This includes your contact information and any important details about your health.

Step 4: Confirm Your Appointment
Once you've filled out the form, confirm your appointment. You'll receive an email with all the details of your appointment. Please check your inbox and make sure to save the date!
Step 5: Complete Forms
Before we finalize your appointment, we need you to complete a few more forms:

Patient Information: Please provide your personal details.

Consent & Privacy Policy: Read through, agree to, and sign our consent and privacy policy.

Insurance Information: Fill in your insurance details, upload a copy of your insurance card to ensure your visit is covered.

Medical History Form: Provide us with your medical history to help us give you the best possible care.

Step 6: Start Your Tele-Med Visit!
When it's time for your appointment, open the link sent to you via email or text message. This link will connect you directly to your online appointment with your doctor. Ensure you have a stable internet connection and a quiet space for the best experience.
Creating Your Passport
After booking your appointment, you'll receive another email titled "Elation Passport Invitation". This is your key to a seamless healthcare experience with us. Follow the instructions in the email to create your passport, which will allow you to manage your appointments, view your medical records, and communicate with our team.
